The failure by maintenance personnel to install a safety lockwire on a fuel line coupling led to the Oct. 29, 2015, fuel leak and fire on a Boeing 767-200ER at Fort Lauderdale-Hollywood (Florida, U.S.) International Airport that seriously injured one passenger and caused substantial fire damage to the airplane, the U.S. National Transportation Safety Board (NTSB) says.
