The final report by Iranian accident investigators says ineffective risk management ‘failed to protect the flight safety’ of a Ukrainian airliner.

Iran’s Aircraft Accident Investigation Board (AAIB) has issued its final report on the Jan. 8, 2020, crash of Ukraine International Airlines Flight 752, officially concluding that an Iranian air defense unit misidentified the Boeing 737-800 and fired the missile that brought it down.
