A “breakdown in foreign object control and tool accountability processes” was to blame for a January 2026 incident in which a Qantas Airways Airbus A380 was flown from Sydney to Dallas-Fort Worth (DFW) and back to Sydney with a work light inside the airplane’s left wing, the Australian Transport Safety Bureau (ATSB) says.
